Permanent Jewelry Deposit

LBM Jewelry Permanent Jewelry Appointment Deposit Agreement

1. Deposit Amount: You agree to pay a non-refundable deposit of $20 to LBM Jewelry to secure the appointment for Permanent Jewelry services at our studio located in Brockton, MA.

2. Appointment Holding: The deposit will secure your appointment for the agreed-upon date and time for Permanent Jewelry services.

3. Deposit Application: The $20 deposit will be applied towards the total cost of the Permanent Jewelry service provided by LBM Jewelry.

4. Late Policy: If you arrive more than 15 minutes late for the scheduled appointment without prior communication with LBM Jewelry, the deposit will be forfeited.

5. Cancellation: In the event of cancellation by you, the $20 deposit is non-refundable. This will only apply if the cancellation occurs less than 24hr from the scheduled appointment.

6. Communication: You agree to communicate any delays or changes in the appointment time with LBM in advance to avoid forfeiture of the deposit.

7. Acceptance: By making the $20 deposit, you acknowledge and agree to the terms outlined in this agreement.
